3. Tell us who this complaint about?
--Examples of players issue: Your game name/id, Server admin, or =PGC= clan member
--Examples of server issue: Server game IP and port
4. What the complaint or issue is about?
--Examples of players issue: Base raping, bad language, being banded from server, cheating, and Admin abuse of power.
--Examples of server issue: Had lots of lag issues (also please tell us your location like Bryan, Ohio USA; your IP; your pings [378]; and how long it lasted like that [20 min]
5. If you could provide proof of the complaint.
--Example: Player X was caught cheating on xyz server using this xyz program and have a video clip to prove it at this url.
